The Title VI formula grant supports the Native American Programs department.

Please find the 506 Form used for Title VI funding and JOM Parental Costs eligibility. There is a sample to guide you on filling out this document, as well as a blank copy. Print and drop off the completed form with front office staff at your school site along with a copy of your student's CIB if requested.
