Welcome to Farmington High School!
This checklist will help you gather documents for the Online Registration.

Step 1: Address Verification
Does the student live within the
district boundaries?

  • ☐ Yes

  • ☐ No

 

If NO:
An approved waiver is required before continuing online registration.
Please complete the waiver application
here (must be approved before proceeding)

 

If YES:
Continue to Step 2

 

Step 2: Proof of Residency

Accepted documents listed below:

  • ☐ Utility or Gas bill

  • ☐ City of Farmington bill

  • ☐ Rental or lease agreement

·         ☐ Current property tax bill

  • ☐ Home Purchase Agreement

 

Important notes:

  • Must be dated within the last 30 days

  • Rental/lease agreements cannot be expired

  • Must show the parent or legal guardian’s name

If the student is not currently living with a parent or legal guardian, additional forms may be required.

Step 3: Birth Certificate

  • ☐ Student birth certificate will need to be uploaded during online registration

 

Step 4: Immunization Records

  • ☐ Current immunization records will be uploaded during online registration

 

Step 5: Previous School Information (If Applicable)

  • ☐ Transcript from previous school

  • ☐ Transfer grades (if applicable)

 

Step 6: Additional Documentation (If Applicable)

If applicable, you may upload any of the following during online registration:

 

  • ☐ IEP or 504 Plan

  • ☐ Court-ordered custody agreement

  • ☐ Guardianship papers

  • ☐ CIB (Certificate of Indian Blood)
