Registration

Please complete New Student Registration if:

• Your child did not attend any Farmington Municipal School during the 25-26 school year.

Make sure you have the following information ready:

• Your child's doctor’s name and contact info
• Health insurance details
• Student health history
• Emergency contact names, addresses, and phone numbers

How to Register:

• Step 1: Click the “New Student Enrollment” box. This will take you to our online registration system, PowerSchool Enrollment (formerly InfoSnap).
• Step 2: Click here to view the list of required documents.

What to do next:

Once you complete the online registration, you should receive a confirmation email indicating that your submission was received.

Our school registrars will return on July 20. At that time, they will begin processing registrations and entering information into the system. If any additional documents are needed to complete your registration, a registrar will contact you directly. You’re also welcome to call the school starting July 23 to check on required documents or confirm any other details.

Current FMS students get automatically re-enrolled. Right before the new academic year, we reach out to families and begin the Annual Student Information Update to make sure we have up-to-date contact information and addresses.

If your student turns 4 before August 31, 2026, please enroll at our new Farmington Preschool Academy (FPA), located at 5840 Fortuna Drive, Farmington!

Peer student program openings for students who are at least 3 years old, and students with IEPs will also be served at FPA. If your child has an IEP and currently attends FPA East, s/he will automatically be enrolled for the 2026-2027 school year.

Non-Public school families, you must complete the Non-Public Student Enrollment & Activities Request and have the form approved before registering online. You can find the form [here]. Below you'll find the required documents and the enrollment process.

Required Documents for School Enrollment
If approved, families must provide the following documents for all students:

• Proof of current immunizations
• Proof of residency
• Proof of age
• Proof of guardianship
• Proof of Tribal Membership

1. Initial Enrollment Requirements
Complete the New Student Enrollment Information form and submit the following documents through the online registration system:

• Proof of residency
• Copy of birth certificate
• Up-to-date immunization records
• CIB (if applicable)

2. Information Submission
Parents/guardians must provide all relevant information about the student, including:

• Medical conditions
• Custody orders (can be submitted in Online Registration)
• Restraining orders (can be submitted in Online Registration)

3. Proof of Age
Present proof of the student’s date of birth to meet the age requirement for Kindergarten or First Grade. Acceptable documents include:

• Original birth certificate
• Hospital record
• Adoption papers
• Custody papers
• Any other suitable document verifying age

4. Immunization Records
Provide up-to-date immunization records or evidence of compliance with New Mexico Administrative Code (NMAC 7.5.2).

5. Verification of Residency

• Homeowners: Acceptable documents include a current property tax bill or utility bill (electric, water, gas, sanitation), purchase agreement, or construction contract.
• Renters: Provide a current rental lease. If the lease is over one year old, include a copy of the lease and a current utility bill.
• Shared Housing: Acceptable documents are a copy of the current property tax bill from the homeowner or a rental lease (with the same conditions as above) from the renter, along with at least two supporting documents.

Approval Process
Families completing this form for courses will be approved and forwarded to the school registrar for enrollment.
For sports, approval from the school's Athletic Director is required before the registrar is notified that the family can complete online registration.
